Air going from your nose out your ears usually happens because your ears and nose connect through a small channel called the Eustachian Tube. 

Normal Anatomy and Pressure

The Connection: The middle ear connects to the back of the nose and throat through the Eustachian tube.

Airflow: When you blow your nose hard, air can travel up this tube and into the middle ear space.

When Air Actually Escapes Out the Ear

Eardrum Perforation: If you actually feel or hear air whistling or rushing out of your ear canal when blowing your nose, it usually means there is a hole or tear in your eardrum (tympanic membrane).

Other Signs: A hole in the eardrum can cause pain, fluid drainage, buzzing, or muffled hearing.
